How do I Look Up Site Information?

To look up site information, click on the "View Data" button on the main menu. This takes you to the View Data page. Alternatively, you can navigate here by clicking on the Audit ID from the Audit List page.

You can search for a site by entering one of the following:

• Site Address

• Site License/Name

• KSB Site No.

If the system finds a match, the corresponding data will load on this page.

The site data includes:

• Clinic Name

• DHCS Site ID

• Address

• Contact Information

• Physician Assignments

• Staff Count (Staffing Matrix)

Below are options to create new audits, and text fields for notes pertaining to this site.

At the bottom of the screen. A list of completed audits at this site is listed. General information is included here such as the date of review and score. You may click on the Audit ID link to view audit details.
