# How do I Look Up Provider Information?

### Looking Up a Provider

To look up a provider, the reviewer can click on the "Provider Info" button.

It is possible to search by provider id, license, or name. Alternatively, by entering the "%" symbol,  the full list of all providers in the system can be retrieved.

After finding the desired provider, click on "Select" to view the provider's information.

#### Information Shown

* Provider ID
* License
* Last Name
* First Name
* Title
* Provider Type (Physician / NPMP)
* Specialty
* Site Status
  * Drop = "No" (Provider is currently assigned to this site)
  * Drop = "Yes" (Provider was previously assigned to this site)
* Start term, end term (if applicable)
* Reapplication date (if applicable)

### Create a New Provider

A new provider can also be created on the Provider Information page.&#x20;

Fill in the following fields:

* Provider Id
* License
* Last Name
* First Name
* Title
* Provider Type
* Specialty

Click on "Save As New" to add the new provider to the system. This provider can now be added to any site.
