Looking Up Sites/Providers

Looking Up a Provider

To look up a provider, the reviewer can click on the "Provider Info" button.

It is possible to search by provider id, license, or name. Alternatively, by entering the "%" symbol, the full list of all providers in the system can be retrieved.

After finding the desired provider, click on "Select" to view the provider's information.

Information Shown

• Provider ID

• License

• Last Name

• First Name

• Title

• Provider Type (Physician / NPMP)

• Specialty

• Site Status

  • Drop = "No" (Provider is currently assigned to this site)

  • Drop = "Yes" (Provider was previously assigned to this site)

• Start term, end term (if applicable)

• Reapplication date (if applicable)

Create a New Provider

A new provider can also be created on the Provider Information page.

Fill in the following fields:

• Provider Id

• License

• Last Name

• First Name

• Title

• Provider Type

• Specialty

Click on "Save As New" to add the new provider to the system. This provider can now be added to any site.
