To generate reports on KSB, click on the "Reports" button on the main menu to access the Report page. Here, you will see a list of reports available to you. Click on the open button to access them. You are also able to download the report in excel format if you wish, by clicking on the excel button after opening the report. Below is a list of all of KSB's reports and functionalities:

Report	Description
Corrective Action Plan (all open CAPs)	Query all open CAPs (CEs and non-CEs)
Initial Reviews due	Query all Initial FSRs and MRRs not yet scheduled or completed
Interim FSRs due	Query all periodic and initial FSR/MRRs that are due for Interim FSR
Periodic FSRs due	Query all periodic and initial FSR/MRRs that are due for their Periodic FSR [3-year, 12-,6-month cycle)
Completed Reviews	Query all periodic and initial FSR/MRRs and PARS that have been completed. Missing either Initial/periodic FSR, MRR or PARS reviews will indicate "N/A" on the report. The report also shows the scores, CAP age, CAP closed dates, review type, etc.
Completed Reviews (recent vs previous score comparison)	Query all periodic and initial FSR/MRRs and PARS that have been completed comparing recent vs previous scores. Missing Initial or Periodic MRRs will indicate "N/A" on the report. The report also shows the scores, CAP age, CAP closed dates, review type, etc.
FSR/MRR Average Scores by County	Query all average scores by county
FSR/MRR Average Scores by State	Query all average scores by state
Periodic FSR Compliance Status	Query all periodic FSR/MRR that are due, scheduled, not scheduled, incomplete, and completed. Includes compliance with timeliness standards.
Poor Performing/Failed Reviews	Query all periodic and initial FSR/MRRs that received a non-passing score. The percentage threshold is customizable (i.e., under 80%, under 85%, etc.).
Compliance (Low) Score Tracker	Query all periodic and initial FSR/MRRs scores (overall and subsection) to compare the last three reviews. To query sites with multiple consecutive non-passing scores.
Criteria Level Compliance	Query the criteria level compliance rates of all periodic and initial FSR/MRRs
Criteria Level Compliance (incl. overall & subsection scores)	Query the criteria level compliance rates of all periodic and initial FSR/MRRs, including the overall and subsection scores.
Query Builder	Create miscellaneous reports using administrative codes
PCP/Specialty Provider Count	Query of all provider specialty count, including PCPs
Interim FSRs Completed	Query all periodic and initial FSR/MRRs that have complete Interim FSR
MCP FSR Assignment(macro level)	Query all contracted provider sites assigned to collaborating MCPs at the macro level. Contains only high-level information such as provider demographic and next review date information.
Audit Tracker	Query all contracted sites that are due for their next Periodic & Initial FSR/MRRs, PARS, and Interim FSRs in a given time period (e.g., calendar year). The report is used to establish MCP Reviewer and FSR Coordinator assignments in Excel. The Excel spreadsheet can be used to populate the FSR/PARS eTool's Template tabs to reduce manual entries and errors.
FSR C (PCP/Spec) Completed	Query all PCPs and specialty provider sites with completed PARS, including accessibility indicators for all sections, total CE missed
FSR D (Ancillary) Completed	Query all ancillary provider sites with completed PARS, including accessibility indicators for all sections, total CE missed
FSR E (CBAS) Completed	Query all CBAS provider sites with completed PARS, including accessibility indicators for all sections, total CE missed
MCP FSR Assignment (micro level)	Query all contracted provider sites assigned to collaborating MCPs at the micro level. Includes provider demographic information, detailed FSR/MRR/PARS and CAP information, comments, etc.
