Looking Up Sites/Providers

To look up site information, click on the "View Data" button on the main menu. This takes you to the View Data page. You can search for a site by entering one of the following:

• Site Address

• Site License/Name

• KSB Site No.

• DHCS Site ID

If the system finds a match, the corresponding data will load on this page.

If there is more than one result, you will be able to select from a list of results.

There will be providers and site results displayed. Select the desired provider/site to view the details for that site. Once a site has been selected, you can view various information such as:

• Site Information (Address, contact, etc.)

• Providers at this location

• Previous Audit results
