MCP FSR Assignment (Micro-level)

The MCP FSR Assignment (Micro-level) report displays all sites contracted with an MCP in a given county, but with much more detail than the macro-level report. To generate the report, select the desired county, and click on submit. The program will download an excel spreadsheet for you.

The excel spreadsheet provides the following information for applicable sites:

• Assigned MCP

• Clinic Information

  • Name

  • Address

  • Contact

  • Main Physician Information

• Prior FSR, MRR, PARS

  • prior date

  • Prior Reviewer

  • FSR/MRR CAP Due

  • FSR/MRR CAP Closed

  • CE Due/Closed

  • Prior Comments

  • Prior Scores

• Current FSR, MRR, PARS

  • Review Date

  • Comment

  • Scores

• Next Survey Date
