This is the 1st CAP/Score Notification section. This displays scores, relevant CAP due dates, and follow-up dates, as well as physician/designee and reviewer signature fields.

In the bottom right corner, the MRR Score overview is shown.

On the bottom, all the physicians and NPMP information may be included as well, by clicking on the "+" sign at the top right corner of this section.
