2. PHI-free MRR

KSB's system will not allow the upload of any eTools with Patient Health Information (PHI) included.

The eTool must be free of any PHI before it may be uploaded.

Before uploading to the KSB system, the reviewer must remove any PHI in the Medical Records Reference tab of the eTool. There is a "-" button on the top right corner of this sheet. Clicking it will remove any PHI records. This will not make any changes to the scoring or the comments entered. The removed records include:

• CIN

• DOB

After removing the PHI, you should save the eTool as a "REDACTED" version, and upload this "REDACTED" version to KSB.
