The FSR Face Sheet mirrors the look of the DHCS FSR Tool cover page. This is the main page of the FSR Section and captures site information from the Template as well as information such as Certification, Provider Type, and Clinic Type.
Another crucial function of this sheet is to capture scores from the FSR scoring sections, as well as display the compliance rate and score results.
These scores are reflective of the individual FSR criteria in each scoring section.
Exempted pass, conditional pass, or fail compliance rates are determined by the scores provided in the FSR criteria. They are also impacted by the scores obtained in the Critical Element criteria. For example, even if the site scored a 99%, and missed one critical element criterion, then a CAP will need to be issued, and the compliance rate will change from exempt to conditional pass.
On the bottom right of this table, you can see the "Next Review Due". This date is generated based on the results of the audit. A 3-year due date if the audit has received a passing score, and a 1-year due date if they failed.
Another very important table here is Table A. This is where KSB and MSRP will upload primary care physicians and NPMP information from. For each person, The name, title, specialty, license #, and provider NPI # are required. You can click on the "+" button to populate existing staff and providers listed in the template. You can also add additional staff by entering the required information.
The rest of Table A is for tracking provider and staff credentials and training, as well as evidence of clinic policies. This is useful, especially when auditing large clinics with multiple staff and providers.
