# 7. Medical Records Reference 🎬

The Medical Records Reference section will store all the patient information (including Private Health Information or PHI) needed for MRR Scoring Sections. It mirrors the look of the DHCS MRR Tool Medical Reference page.

The required fields include:&#x20;

* CIN
* DOB
* Age
* Gender
* Member’s health plan code/name
* Member’s MCP Enrollment or PCP Effective Date

The age is automatically calculated based on the DOB entered.

The KSB system will not allow any tool with PHI to be uploaded to its system. Therefore, when uploading the completed audit, a separate "PHI-free" or "REDACTED" version of the eTool must be uploaded instead.

### **Removing PHI**

After completing the audit, and before uploading to the KSB system, you must remove any PHI records from this section. There is a "-" button on the top right corner of this section. Clicking it will permanently remove all PHI records. This will not make any changes to the scoring or the comments entered. The permanently removed records include CIN and DOB.

After removing the PHI records, save the eTool as a "REDACTED" version, and upload this redacted version to KSB.<br>
