The MRR Face Sheet mirrors the look of the DHCS MRR Tool cover page. This is the main page of the MRR Section and captures site information from the Template, as well as additional information such as Certification, Provider type, and Clinic Type.
The provider name, credentials, and Provider NPI table may be left blank since KSB captures the information from Table A in the FSR portion. The MRR Face sheet is almost identical to the FSR Face sheet, with the exception of Medical Record information.
Another crucial function of this sheet is to capture scores from the MRR scoring sections and display the compliance rate and score results.
These scores are reflective of the individual MRR criterion in each scoring section.
Exempted pass, conditional pass, or fail compliance rates are determined by the scores achieved in the MRR scoring sections.
On the bottom right of this table, you can see "Next Review Due". This date is generated based on the results of the audit. A 3-year due date if the audit has received a passing score, and a 1-year due date if they failed.
